MCLC Employee BLS Registration Form

If you are taking ACLS or PALS the same day, please do not use this form. You will be given the option for BLS in the ACLS or PALS registration form. This form is for BLS only and you are not taking ACLS or PALS the same day.

It is now MANDATORY that you complete the HeartCode BLS online course prior to attending your Skills Testing session. You will receive a "course key" (by email), along with instructions on how to access this online course, usually within 24 hours (excluding weekends & holidays). Please allow enough time to complete this online course prior to the date of your Skills Testing.
  • The HeartCode BLS online course usually takes 60-90 minutes to complete, depending on your skill level.
  • You must bring a copy of your course completion certificate with you to the Skills Testing session for admittance.
  • This completion certificate is only valid for 60 days from date on the certificate. After this, you will need to complete the course again.
  • The BLS Skills Testing starts promptly at 7:30 am. If you do not arrive by 7:30 am, you may be asked to reschedule your skills testing for another date.

By checking this box, you understand that you must complete the AHA HeartCode BLS online course and bring a copy of course completion certificate to the Skills Testing session you will register for below. Your BLS certificate is only valid for 60 days from the date on the certificate.

  • A copy of this form will be sent to the Medical City Las Colinas Education Department for approval.
  • After submission, you will receive an email confirmation. Please be sure to verify that all the information is correct.
